Employee Health Plan Coverage
ATU Represented Employees
ATU represented employees pay $35 per month for their medical coverage. Effective 6/11/12, ATU represented employees will pay the larger of $35 or any premium in excess of the Kaiser family premium.
Administrative Employees
Administrative Employees (AFSCME, SEIU, TAEA and Non-Represented employees) pay a $35 per month contribution for their medical coverage plus any premium in the excess of the Kaiser family rate.
Administrative employees must work a minimum of 20 hours per week to be eligible for benefits. Part-time employees (working less than 32 hours) pay a pro-rated share, depending on the number of work hours.
Health Plans
Medical Opt-Out - Full-time employees may opt-out and receive a taxable monthly payment if they have other coverage. Employees who have a spouse or registered domestic partner working at VTA are not eligible for Medical Opt-Out.

Kaiser Permanente HMO (www.kaiserpermanente.org) $5 Office visits; $5 for 100-day supply prescription drugs, $50 emergency room visit (waived if admitted)
PacifiCare HMO (www.pacificare.com) - $5 Office visits; 30-day supply prescription drugs - $5 for generic, $10 for brand; $20 for non-formulary; $50 emergency room visit (waived if admitted)
United Health Care-Choice Plus (www.uhc.com) (administrative employees only) co-pays and deductibles vary depending on whether care is accessed in network or out-of-network
Vision Service Plan (www.vsp.com)
ATU Represented Employees - Eye exam and lenses every 12 months; frames every 24 months; total deductible $10
Administrative Employees - Eye exam every 12 months - $20 deductible; lenses & frames every 12 months - $20 deductible
Pacific Union Dental (www.pacificuniondental.com) (administrative employees only)
Pays 100% for most services. Co-pay and upgrade fee for others.

ATU Represented Employees - 90%/10%; no deductible, $1,500/yr max per individual
Administrative Employees - 75%/25%; no deductible, $2,000/yr max per individual
