Entry forms must be submitted or postmarked by July 16, 2010 to be eligible for the drawing. No purchase necessary. Entry forms may also be mailed to VTA Customer Service, 3331 N. First Street, Bldg. B, San Jose, CA 95134-1927. Limit one entry per person. Entries that are incomplete or illegible will not be accepted. At the conclusion of the promotion, 10 Westfield Gift Cards in $100 denominations will be awarded. No substitutions or exchanges are permitted. The random drawing will be held on August 6, 2010. Prizes will be awarded only upon winner verification and final approval. Winning recipients will receive prize notification via United States Postal Service and will have (30) days from the postmark of the winning notification letter to claim their prize. If winning entrants of any prize cannot be contacted, fails to claim the prize, is ineligible, or if the winning notification is returned undeliverable, he/she will forfeit that prize and the prize will be awarded to the next randomly drawn winner. Acceptance of a prize constitutes permission for VTA to use the winner’s name for advertising and promotional purposes without compensation.
